Referral network overview
Beautifying access to community services.
Navigating the complicated health care system is challenging. Patients, caregivers and clinicians find it difficult to understand what organization provides which services, where, and whether a person is eligible for service. Connecting to these services can be even harder, and people are often left wondering who is going to help them, and when.
For the purposes of the "Let's Go Home" or "LEGHO" program, AbleLiving Services / Thrive Group is bringing organizations together to transform how patients going home from hospital are connected to local CSS services, ensuring they remain well supported post-discharge and reducing readmissions to hospital.
By leveraging available digital health tools available in the region, AbleLiving will be launching a new website with a 24/7 eReferral portal, making it easy for the members of the public, hospital discharge staff and other approved referrers to send a service request to LEGHO Community Connectors and the trusted members of the HNHB Community Support Services network (HNHB CSS). A secure back-end interface will be available for HNHB CSS partner organizations (you!) to receive and manage inbound requests, optionally publish available appointment times and report on your patient's outcomes.

The Community Support Finder is a partnership of local health care organizations and providers who are committed to ensuring patients, families and caregivers in the HNHB region have better and more seamless access to the care they need, when and where they need it.

What is Caredove?
The platform is built on Caredove.
Caredove is an Ontario-based eReferral and access management software platform that creates trusted networks of home and community services. Clinicians can search for services, book appointments and manage eReferrals within Caredove. Caredove also builds secure websites that help 1000+ organizations market their services and receive public requests through a responsive eReferral portal.

Signing up to Caredove is simple.
If you are new to Caredove, the onboarding & training process takes approximately 1-2 hours per organization.
- Fill out the form with your basic organization information.
- Meet with Tina, the Caredove Operations expert who will meet with you virtually to get you familiar with Caredove.
- Learn how to create service listings, build an intake appointment schedule and manage your inbound referrals.
If you are already using Caredove to receive referrals, you may not require any additional training. Tina will be in touch and answer any questions you may have.
